Annual Wellness Visits, Preventive Care

Annual Wellness Visit and Physical Examination
The Annual Wellness Visit is a yearly appointment with your primary care provider to create or update your own personalized health plan. The Annual Wellness Visit requires a review of your Past Medical History, Family History, Social History, and medications; an assessment of health risk factors, such as, blood pressure, weight, tobacco usage, mood disorders; and a discussion on diet, exercise and lifestyle issues. Current medical problems are also evaluated and managed. And when appropriate, the visit also covers immunizations and screening for colon, breast and prostate cancer.

Lifestyle Modification
Altering long-term habits can help you improve many health conditions such as, blood pressure, obesity, diabetes, cholesterol, anxiety, depression, stress, sleep issues, tension headache, irritable bowel and many other issues. Diet, nutrition, exercise, mindfulness, Cognitive Behavioral Therapy, and Action Behavior Therapy are examples of lifestyle modifications that are being successfully used to manage the above conditions. These are important and proven therapies to consider before using medications.

Diagnosis, Disease Management

What distinguishes our practice from most other Primary Care Medical Practices, is that we’re led by a Board Certified Internal Medicine Physician who has extensive training in the diagnosis and management of both simple and complex medical problems.

We help you understand your condition and make decisions about your care, while explaining treatment options that make sense for you.

Following is a list of some of the more common health issues we frequently diagnose, treat and manage, but there are MANY more diseases we treat on a daily basis.

Hypertension
Hypertension is a condition when blood pressure is too high, and above recommended levels for good health. It’s important to control your blood pressure to reduce your chance of future heart-attack, congestive heart failure, stroke, chronic kidney disease and memory loss leading to dementia. There are now available various types of generic medications with very few, if any, side effects to make sure blood pressure is controlled. For those who have a problem, it’s helpful to monitor blood pressure outside of the office environment.

Hypercholesterolemia
Hypercholesterolemia or High Cholesterol is an excess of cholesterol in the blood stream. If untreated, high cholesterol can lead to build-up of plaque and blockages within your arteries. Other risk-factors such as, age, gender, blood-pressure, diabetes and cigarette smoking, need to be taken into account before medications are used, if diet alone is insufficient.

Obesity
Obesity is a condition when a person carries excess weight that could lead to many different types of health issues. Nutrition, diet modification, exercise, and behavioral changes are all important factors that are usually addressed. We understand this can be a challenging issue for some, and are here to help.

Type II Diabetes Mellitus
Diabetes occurs when patients lose more than 80% of insulin-producing cells within the pancreas that are sufficient to control blood-sugar. Uncontrolled diabetes leads to damage of the heart, circulation, kidney, eyes and nervous tissue. Risk factors include family history of diabetes and obesity. Pre-diabetes precedes diabetes, and can be detected and treated to delay the onset of the disease. There are now new medications available to delay the loss of pancreatic insulin-producing cells. There are new ways to be able to continuously monitor blood sugar using a skin patch.

Anxiety
Anxiety is common and is the body’s response to stress. A person can feel worried, apprehensive or fearful. Mindfulness which can be learned through the use of various Apps on a regular basis has been shown to be very effective for a lot of people. Cognitive or Action Behavior Therapy, spirituality, exercise, diet are also helpful. Medications are sometime required.

Depression
Depression is a mood disorder that causes a persistent feeling of sadness and loss of interest. Screening, questionnaires can be used to determine the severity of depression and also to follow the progress with treatment. Lifestyle modifications including mindfulness, behavioral therapy, exercise and sleep, can be very helpful. Very effective medications are also available.

Attention Deficit Hyperactivity Disorder (ADHD)
Attention Deficit Hyperactivity Disorder is characterized in adults by a difficulty in focusing and completing tasks. Up to 50% of people diagnosed in childhood continue to have symptoms in adulthood. Up to 5% of the adult population have this condition. Treatment may involve a combination of counseling, lifestyle changes and medication.

Thyroid Disease
The thyroid gland in the neck produces hormones that help regulate metabolism. Hypothyroidism is when the thyroid doesn’t produce enough hormone, and hyperthyroidism is an over-production of hormone. This can lead to specific symptoms. The pituitary gland in the brain, responds to thyroid hormone levels and measurement of thyroid stimulating hormone (TSH) helps in the management and treatment of disease.

Coronary Artery Disease
Atherosclerosis, which is the build-up of cholesterol and plaque, leads to narrowing or blockage of the arterial blood vessels that supply blood and oxygen to the heart muscle. This can lead to a type of chest pain known as angina pectoris, and sometimes to a heart attack. The reduction of risk factors such as hypertension, hypercholesterolemia, obesity, diabetes mellitus and cigarette smoking, along with lifestyle modifications that include diet, exercise and stress reduction, are all very important. In non-emergent situations, medications have been shown to be as effective as invasive procedures.

Congestive Heart Failure
Poor contraction or relaxation of the heart muscle can affect the ability of the heart to pump blood and lead to a build-up of fluid in the lungs and body tissues. Poor function of the heart valves can also lead to congestive heart failure. Lifestyle changes such as exercise, sodium dietary reduction, weight control and stress reduction are helpful. Medical management of hypertension, coronary artery disease and diabetes are important. Additionally, medications that improve the ability of the heart muscle to perform more effectively are available.

Coordination of Care

After our thorough evaluation of your health, our team determines when it’s necessary to bring in a specialist to either consult or co-manage your medical problem. We also make sure to keep up with your medical care that happens outside of our office, while helping to provide additional understanding and advice when needed.

For Medicare Patients

Chronic Care Management is a service we offer patients age 65+ to provide a higher level of care for better management and coordination of their health care needs. Our nurse who heads up the program, is in contact with patients through regularly scheduled phone calls, to monitor any changes in health conditions and medications. The service is covered by Medicare and Medicare Advantage plans.

We’re a nationally certified Level 3 PCMH practice, the highest level available. PCMH is a team-based model of primary care emphasizing coordination and communication, to optimize your care. We’re one of the first independent practices in the DFW area to receive this important designation.
